Quarterly Reports – The Statistical (comprised of the Statistical page and the Narrative page) and Financial Reports are due in the UBF each quarter on the dates shown below.  Both reports must be submitted on the forms provided by the UBF and should be submitted together.

Agencies receiving one-time allocation checks should follow the same reporting schedule as those receiving quarterly disbursements unless advised differently by the Director of Programs and Services.

Delinquent Reports – An agency that is one month delinquent in submitting the required quarterly reports will not receive that quarter’s allocation until the next quarter disbursements are issued and the offending agency is compliant.

Agencies that have two are more reports in arrears are in jeopardy of having their future disbursements suspended or revoked for the balance of the funding period.
